Taichung Veterans General Hospital (TCVGH) has established an integrated care system for osteoporosis in 2017, integrating the comorbidity risk of osteoporosis and the 14 departments of high-risk group frequent visits, and timely providing screening and treatment of osteoporosis fractures, including department of orthopedics, neurosurgery, immunity & rheumatology, neurology, chest medicine, metabolism, rehabilitation, nephrology, gynecology geriatric, family medicine, breast Surgery, urology and traditional chinese medicine. The omni-directional interdisciplinary osteoporosis team cooperates to introduce the concept of prevention for the general public and osteoporosis patients, and regularly organizes professional education and training to provide patients with osteoporosis with good service quality.
Through information system referrals, hospitalized patients with suspected fragility fractures will start informational notification at the same time of admission. After the doctors prescibe the drugs, they can refer them to case manager for osteoporosis education, and the case manager will use the information system to track the patient's return to the clinic or not. Besides, the case manager will conduct telephone care counseling to improve medication compliance.
In order to detect the risk of high risk of osteoporosis patient early, TCVGH will continue to conduct screening activities in outpatient clinics at specific festivals . Through multi-disciplinary organized patient education lectures and activities, we are eager to introduce the importance concept of prevention of osteoporosis; provide "integrated group education" twice a year. A team of case manager, pharmacists, dieticians and physiotherapists had provided expertise to enable people to obtain comprehensive osteoporosis medical information through group education.
After the integration of various services of medical department in 2017, we will build a comprehensive information system, through early screening and arrange seminars, regular medical staff training and monitoring of medical quality, and hope that the disease will be implemented in various subjects. Goal of this project is improving the quality of the entire medical treatment. After the osteoporosis cross-team was established, there is a significant improvement in bone density examination, medication compliance and overall medication. It is hoped that through the continuous efforts of the team, the future will provide better medical care for the people.
